The communities we serve, with
whom we work, and from whom we gather strength - while separate and
distinct from one another - are nonetheless bound by a single common
interest. That interest is improved care at the end of life.
TrinityCare Hospice cares, first and
foremost, for a community of patients and their families. The unmet
needs of those who are dying and those who care and grieve for them
motivates our work.
We are also
a community of hospice team members - doctors, nurses, clinical
social workers, chaplains, therapists, home health aides, and
bereavement specialists - whose comforting care improves the quality
of life for our patients. This community would be incomplete without
our hospice volunteers who give selflessly of their time to help
others.
Furthermore, we know
that no hospice could survive without its community of donors whose
empathy or personal experience with hospice leads them to enrich and
sustain TrinityCare Hospice through their financial support. This community
of volunteers and benefactors - including TrinityCare Hospice
Foundation, which helps to fund TrinityCare Hospice's services - is our
life force.
TrinityCare Hospice
has effective agreements and partnerships with a community of
healthcare providers. Our collaboration with physicians, hospitals,
nursing homes and health insurers provides access to hospice for
patients who would benefit from services. We work together to make
TrinityCare Hospice care available to everyone who wants it in
Los
Angeles
and Orange
counties.
Finally, through the National Hospice & Palliative
Care Organization, Southern California Cancer Pain Initiative, the California Hospice &
Palliative Care Association, and CHAP certification, TrinityCare
Hospice belongs to a national community of hospice related organizations. This kinship
leads to a seamless exchange of information and education, improving
hospice care for all Americans.
