A recognized leader
and innovator in hospice and end of life
care.
Established in 1977,
TrinityCare Hospice is the oldest and largest non-profit hospice in
Southern
California and the first to be
licensed by the State of California.
Caring for people in Los Angeles and Orange Counties with
professional teams in Torrance, Van Nuys and Cerritos,
TrinityCare Hospice is Medicare Certified and CHAP (Community Health
Accreditation Program) accredited. We also provide the only
specialized pediatric hospice service in
Los
Angeles
County.
The heart of TrinityCare Hospice is the
team: a group of professionals who provide comfort and care to those
individuals for whom a cure is no longer possible. Comprised of the
patient's physician, a medical doctor, registered nurses, clinical
social workers, non-denominational chaplains, home health aides,
registered dietitians and trained community volunteers, the
TrinityCare Hospice team works together with the patients and their families to
make the individual's remaining months and weeks of life as
meaningful and comfortable as
possible.
While the primary goal
of health care is to cure disease, there are times when this is no
longer possible. When the focus of care shifts from cure to comfort,
TrinityCare Hospice offers the expert medical care and human
compassion needed by both patients and families. We are
life-affirming - encouraging patients to live the final end stages
of life instead of merely enduring them. As pain is relieved and
symptoms are kept under control, the patient is better able to
participate in daily life at home with family and friends. Our two
top priorities are: relieving pain and creating opportunities for
open communication.
