The TrinityCare Hospice team provides comfort and care to individuals and their caregivers for whom a cure for illness is no longer possible. The top two priorities of hospice care are relief from pain and other distressing symptoms and creating opportunities for communication and closure. In order to fulfill these goals, the TrinityCare team is dedicated to making each patient's remaining months and weeks of life comfortable and meaningful. They work hand-in-hand with patients and their families to achieve this.

Each patient's situation is different. TrinityCare Hospice provides a variety of services so that any and all needs of our patients and their families are met. Many of these services are listed below.

Specialized services focus on pain relief, symptom management, comfort and support for terminally ill patients at home, in nursing homes or assisted living facilities.

TrinityKids Care serves infants and children under age 18

Care is provided by an interdisciplinary team of hospice-trained professionals including physicians, registered nurses, clinical social workers, non-denominational chaplains, home health aides, physical and occupational therapists, dietitians, and specially trained community volunteers

24-hour, seven-day availability of a hospice nurse, physician and clinical social worker

Regular professional visits

Medical supplies and equipment

Personal care

Full-service pharmacy that can deliver medications around the clock

Instruction in care-giving to help family and friends care for their loved one

Inpatient respite care

Support and practical assistance from trained hospice volunteers

Emotional and spiritual support for patients, their families and friends

Bereavement support for surviving family members, friends and caregivers for 13 months following the death of the patient, including age-appropriate support groups for adults, adolescents and children.
